IMM 5744 Form: What It Is and How to Complete It (2026)

The IMM 5744 form is IRCC's consent form that lets you authorize IRCC to release your personal information to a person you name. You do not need it to get your own immigration file. You need it only when someone else, like a family member or a representative, is requesting your information for you.
That one distinction is where the confusion starts, and getting it wrong is the difference between a request that moves and one that sits. Forget a consent form for a spouse or an adult child listed on the file and IRCC cannot release their part of the record. The official IMM 5744 page is the form itself; the sections below cover what that page does not explain plainly.

What is the IMM 5744 form?
IMM 5744 is the Consent for an Access to Information and Personal Information Request. In IRCC's words, it is "for people who wish to authorize the Access to Information and Privacy (ATIP) Division at IRCC to release their personal information to an individual that they designate."
Think of it as a permission slip. You are the person whose file is being requested, and you sign IMM 5744 to tell IRCC it may hand your information to the specific person you name on the form. It travels with the request, never on its own.
When you actually need IMM 5744 (and when you don't)
Your immigration file works like your own medical records. You can ask for a copy of your own records without anyone's permission, but if your sister wants a copy of yours, the clinic needs you to sign something first. IMM 5744 is that "sign something first."
So the rule is simple: requesting your own information, you do not need IMM 5744; someone requesting on your behalf must include a signed IMM 5744 for you. The two request routes also cost different amounts. A request for your own personal information under the Privacy Act has, in IRCC's words, "no fees." A broader request under the Access to Information Act costs $5.00. The consent form comes in when a representative or relative pulls your file, often to review a refusal or prepare a fresh application.
How to complete and where to send the IMM 5744 form
You fill in the details of the person designated to receive your information, then date and sign to give consent. Include a separate IMM 5744 for each individual on the file whose information is being requested.
Here is the detail people miss, and the reason searches like "can a 17-year-old sign the ATIP consent form" exist: the age at which a person signs their own consent depends on the Act. On a Privacy Act request (your own information), IRCC asks for a consent form for each individual 16 years of age or older. On an Access to Information Act request, the threshold IRCC states is over the age of 18. For a child under that age, a parent or guardian signs.
To submit, apply online through IRCC's ATIP portal, or mail the completed request, along with a request form such as IRCC's IMM 5563 (a plain letter is also accepted), to the Access to Information and Privacy Division in Ottawa.

IMM 5744 vs IMM 5475 vs IMM 5476
Three IRCC forms sound alike and get mixed up constantly. IMM 5744 is the ATIP consent form: it lets IRCC release your existing records to someone for an access request. The IMM 5475, Authority to Release Personal Information to a Designated Individual, lets a person you name contact IRCC and receive information about your live application without becoming your representative. The use of a representative form (IMM 5476) goes further, authorizing someone to actually represent you and conduct business on your file.
